Eligibility Screening
Initial Contact
Telephone and/or Face-to-face
A resident of Linn County wishing to speak with a Crisis worker and/or inquire about mental health services with Linn County Mental Health can call or come to the Albany Walk-In Clinic to discuss eligibility, resources and options.
- Onsite walk-in services are available Monday through Friday from 8:30 a.m. to 5:00 p.m. at 445 Third Avenue SW; Albany, OR 97321
 - The same services are available by phone, call 541-967-3866 (press 0)
 - If you would like to speak with a crisis worker face-to-face, then you will want to arrive before 3:30 p.m. to allow you time to complete important paperwork.
 
You will be asked to provide initial information, which may include:
- Your name
 - Date of birth
 - Social security number
 - Address
 - Phone number
 - Insurance information
 - Brief statement of your request (why you are seeking services)
 
Based on your request, then you may be connected with a crisis worker at that time, or a crisis worker will need to call you back to discuss eligibility, resources and options.
At the walk-in clinic, you will be asked to complete Linn County Mental Health's initial paperwork prior to an in-person meeting. This may include:
- Consent to treat
 - Information sheet
 - Release of information to your primary care physician and insurance company (if applicable)
 - Fee Agreement based on a sliding fee scale
 - Provided the "notice of Privacy Practices & Client Rights and Responsibilities"
